Mister X is the head of an international smuggling ring in Chicago and arrives in Cairo to practice his activity. The police know about him and begin to watch him. The film is an adaptation of the novel Samar Habib by Ismåeel Walieddin. author of Female Homosexuality in the Middle East: Histories and Representations. The main character, Ahmad, leaves rural eastern Egypt for the city hoping to become economically self-sufficient, gets an apartment for his parents, and obtains a law degree. He and his family are refugees from a town occupied by the Israeli army, Ismaåilia. Ali, the owner of the Malatily Bathhouse, offers to let him stay there for free. Ahmad encounters several characters there, including Naåeema, a prostitute who he becomes obsessed with, and Raouf, a homosexual man.

On a fishing trip, Adel takes his three friends, and while fishing they find a heavy bag. The three rush to pick it up, and find a shipment of drugs inside. Three gang members watch the situation from a distance, and when the friends reach the bag, the gang members intervene to seize the bag. At this point, police officer Lotfy appears and tries to arrest them.

A meeting with a new inmate in the psychiatric hospital flips Dr. Yehia's life upside down. She prophecies that the death of his entire family is only three days away.
